Child Protection Index Check Forms from Other States - ALL SCHOOL APPLICANTS/EMPLOYEES who have lived outside of Indiana within the past FIVE (5) years.
As of July 1, 2018, an expanded child protection index check may be run as found at IC-20-26-2-1.3, which consists in part of an inquiry with the department of child services of any state in which the individual has resided since the age of 18, as to whether an individual has been the subject of a substantiated report of child abuse or neglect.
The Indiana form (found on the instructions page) is required for all applicants. If you have resided in a state other than Indiana within the past five (5) years, the following nearby states' forms are provided for your convenience to be returned to the Catholic Schools Office.
Instructions on these forms may vary, but please fill them out as instructed and send them to the Catholic Schools Office at [email protected], so that we can complete our portion of the form before submitting them. Do NOT send them directly to that state's department of child services; instructions to do so on the individual forms are directed to the schools office, not to the applicant.
