Bus Driver Application Instructions

Thank you for your interest in driving a bus for the Catholic schools in the Diocese of Evansville.

We ask that all those intereseted in becoming a bus driver complete the application packet found to the right.  The completed application packet materials must be submitted to cso@evdio.org. The Catholic Schools Office maintains a master file of bus drivers.   Principals will be notified of complete applications on file.  Bus drivers are called on an as-needed basis by the individual schools.  Pay varies at each location, and I-9 and W-4 forms will need to be completed at each employing school.  

Applicant files will be kept on file for one year, unless you annually request that it remain active. Notification is greatly appreciated if you accept another position or if you no longer wish to be considered as a candidate for employment.  

As the saftey of students, teachers, and even parents, is upon the shoulders of those who transport our students, all bus drivers are required to participate in the Drug and Alcohol Testing Program for the Diocese of Evansville.

The following items must be included in the application packet returned to the Catholic Schools Office at cso@evdio.org.
  • Bus Driver  Application
  • ​Child Protection Index Check Form (IN)
  • Child Protection Index Check Form from Other States Resided ​
  • Copy of Medcical Examiner's Certificate of Driver's Physical Qualifications
  • Copy of Driver's License
  • Copy of Social Security Card
  • Copy of Yellow Card
  • Road Test/ Bus Safety Test
  • Previous Employer Request for Information
  • Pre-employment Urinalysis Notification
  • Drug and Alcohol Testing Policy (for reference)
  • Drug and Alcohol Testing Policy Acknowledgement & Addendum
  • Drug and Alcohol Testing Notification & Consent Form
  • Drug and Alcohol Training Packet (for reference)
  • Drug and Alcohol Training Acknowledgement
